Prior to evaluation and therapeutic services, a consultation appointment should be scheduled. This initial consultation will allow you to convey a complete picture of your concerns in order to determine the appropriate next steps.
To help us achieve the initial evaluation goals, please provide Comprehensive SLP with the following information:
- General background information
- Previous and current Individual Educational Plans
- Prior independent evaluations and therapy progress reports
- Contact information of those professionals who are currently part of your child’s therapeutic and educational team.
